铁蛋白与2型糖尿病胰岛素抵抗的性别差异性研究(1)
[摘要]目的 分析2型糖尿病(T2DM)患者血清铁蛋白(SF)水平与胰岛素抵抗(IR)及性别差异的相关性。方法 选取我院2014年7月～2015年12月在内分泌科住院的T2DM患者49例，根据性别分为男性组25例和女性组24例。另选取同期正常体检者50例作为对照组。检测各组的总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、超敏C反应蛋白(CRP)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、血清铁蛋白(SF)及空腹血糖(FPG)水平。结果 T2DM组的TC、TG、LDL-C、HDL-C、CRP、HbA1c、FBG、FINS、SF及稳态模型评估胰岛素抵抗(HOMA-IR)水平明显高于对照组(P<0.05)，SF与TC、TG、LDL-C、FBG成正相关(P<0.05)，SF与HbA1c、HOMA-IR、FINS、CRP成明显正相关(P<0.01)。男性HOMA-IR与SF的相关性比女性HOMA-IR与SF的相关性更好。结论T2DM患者SF水平升高，并与IR有一定的相关性。男性T2DM组的HOMA-IR 与SF的相关性更好。

[Abstract]Objective To investigate the correlation between serum ferritin (SF) and insulin resistance and its sex differences in patients with type 2 diabetes mellitus (T2DM).Methods 49 patients with T2DM treated in our hospital of department of endocrinology from July 2014 to December 2015 were selected.According to the sex，patients were divided into the male group including 25 patients and the female group including 24 patients.50 healthy subjects receiving physical examination at the same period were included in the control group.Total cholesterol (TC)，triglyceride (TG)，low-density lipoprotein-cholesterol (LDL-C)，high-density lipoprotein-cholesterol (HDL-C)，C-reactive protein (CRP)，HbA1c，fasting insulin (FINS)，SF，and fasting plasma glucose (FPG) were detected in various group.Results TC，TG，LDL-C，HDL-C，CRP，HbA1c，FBG，FINS，SF，and homeostasis model assessment-insulin resistance (HOMA-IR) were significantly higher in the T2DM group than in the control group (P<0.05).SF presented positive correlation with TC，TG，LDL-C，and FBG (P<0.05)，and significant positive correlation with HbA1c，HOMA-IR，FINS，and CRP(P<0.01).The correlation between HOMA-IR and SF was higher in male patients than in female patients with T2DM.Conclusion SF is elevated in patients with T2DM，and to some extent correlated with insulin resistance.The correlation between HOMA-IR and SF in male patients is more better.

糖尿病是当前威胁全球人類健康的最重要的非传染性疾病(noncommunicable diseases，NCD)之一，在我国，成年人2型糖尿病(T2DM)患病率已经高达9.7%，高达9240万[1-2]，患病人数居世界首位，糖尿病的防治形势非常严峻。而糖尿病的危害来源于其慢性并发症及相关大血管疾病。胰岛素抵抗和胰岛β细胞功能缺陷是2 型糖尿病的主要发病机制[3-4]。本研究通过分析铁蛋白在不同性别的T2DM患者中的水平，探讨其与胰岛素抵抗的关系，为临床上伴有胰岛素抵抗的T2DM患者药物的选择提供参考依据。 (张玮)
