改良双眼外直肌后徙手术对大角度外斜视矫正的效果研究(1)

     [摘要]目的:探究改良雙眼外直肌后徙术对大角度外斜视的矫正效果,为患者的治疗提供新型手术支持。方法:选择2015年7月-2018年7月笔者医院收治的160例大角度外斜视患者作为研究对象。按照入院先后顺序,将其均分为观察组和对照组。对照组实行双眼外直肌等量后徙术,观察组则在此基础上对术式进行改良,适当增加了外直肌后徙量。比较分析治疗前后两组患者的斜视度、眼位矫正情况、眼球内转受限情况以及双眼视觉功能。结果:治疗后1个月、3个月和6个月,观察组患者的斜视度均明显优于对照组,差异有统计学意义(P<0.05);治疗后6个月,观察组眼位正位率为92.50%,明显高于对照组的73.75%,其欠矫率和过矫率均显著低于对照组,同时其眼球内转受限率(3.75%)也明显低于对照组(13.75%),组间比较差异有统计学意义(P<0.05);观察组患者的立体视恢复率和融合功能恢复率分别为91.25%、98.75%,均明显高于对照组(71.25%、91.25%),差异有统计学意义(P<0.05)。结论:改良双眼外直肌后徙术在矫治大角度外斜视上效果显著,能够有效纠正大角度外斜视患者的眼位和斜视度,其效果较好,有利于患者视觉功能的改善和恢复。

    Abstract: Objective To research the effect of modified bilateral lateral rectus recession surgery on correction of large-angle exotropia. Methods 160 patients with large-angle exotropia who admitted to our hospital from July 2015 to July 2018 were selected as the research objects. According to the admission sequence, they were divided into the observation group and the control group. The patients in the control group were treated with equal volume migration of the external rectus muscles, while the patients in the observation group were improved on this basis, and the recession of lateral rectus muscle was increased appropriately. The strabismus, eye position correction, intraocular rotation limitation and binocular vision function of the two groups were analyzed and compared before and after treatment. Results After 1 month, 3 months and 6 months of treatment, the strabismus of the observation group was significantly better than those of the control group, and the differences were statistically significant(P<0.05). 6 months after treatment, the ocular positioning rate of the observation group was 92.50%, which was significantly higher than 73.75% of the control group, the undercorrection rate and overcorrection rate of the observation group were significantly lower than those of the control group(P<0.05). At the same time, the rate of limited intraocular rotation (3.75%) was also significantly lower than those of the control group (13.75%, P<0.05). The recovery rates of stereopsis and fusion function in the observation group were 91.25% and 98.75% respectively, which were significantly higher than those in the control group (71.25% and 91.25%, P<0.05). Conclusion The improved lateral rectus muscle migration is effective in correcting large angle strabismus, and can effectively correct the eye position and strabismus of the patients with large angle exotropia. The effect is better than that of conventional bilateral lateral rectus recession, which is conducive to the improvement and recovery of visual function of patients., 百拇医药(刘璐 崔茜)
